Please answer the questions below as thoroughly as practical, and include any pertinent comments for help in returning your system to proper working order. Mystifier II/III (Pump models 908/910/914).
1. Is the Pump humming in the Auto position? |

5. If the Pump does not operate in Auto position, does the Pump operate in Manual position? |
|

| 6. If the above answer is yes with Pump Model 914, are the nuts on the Temperature Switch Clamp extremely tight? |

| 7. Is the level of the oil in the tubing above or below the Pump? |
|
|
| 8. If the level of the oil is below the Pump, has the Pump been re-primed according to instructions? |
|
|
| 9. If the level of the oil is above the Pump, is oil pumped out at a normal rate when all the discharge tubing is removed? |
|
|
| 10. Does the oil pump through the Check Valve? |
|
|
| 11. Does the oil pump through the Injection Valve? |
|
|
| 12. Is there evidence that steam has entered the discharge tubing? |
|
|
| 13. Has the Pump been dropped, or fallen to the floor due to vibration? |
|
|
| 14. Is the Foot Valve/Strainer at the bottom of the bottle in an upright position, and held in place by the ¼” clamp on the tubing above the bottle cap? |

| 15. How long since the Bullet Valves have been replaced? |
|
|
| 16. Have the Bullet Valves ever become disassembled? |
|
|
| 17. Are arrows on both Upper & Lower Bullet Valves pointed upwards? |
|
|
| 18. Steam room thermostat is programmed to cycle in |

|
| 19. Apparatus used at point of injection? |
|
|
| 20. Is the vertical rise from the pump to injection point greater than 10ft? |
